Will gummy bear implants give a more natural breast shape?

Gummy bear or anatomical implants are shaped like a breast. Hence they have texturing on the implant surface to adhere the implant to the breast tissue to prevent rotation. If these implants rotate, it produces an unsightly appearance as the fuller portion of the implant is now higher up. Traditional silicone implants are hemispherical but assume a breast like shape when the person is in an upright position. Hence even if the implant flips around, it is of no consequence as it still maintains its shape. Given its natural shape, there is a tendency to think that anatomical implants are better. However a recent study found that plastic surgeons were only 50% correct in predicting if a woman had anatomical or traditional implants when shown a photograph. Traditional implants have the same width and height; anatomical implants can be shaped such that they can be wider than tall, taller than wide etc. In most patients this is of no consequence. However, uncommonly some women an have unusually shaped chest wall where a shaped implant is preferable. Besides, texturing on the implant surface has been related to a rare form of cancer called ALCL (Anaplastic large cell lymphoma). This has not been observed with smooth surfaced implants. The verdict: In most women traditional implants give as good a result as anatomical implants without the risks of rotation and ALCL. Do I need a midface lift? Will a facelift correct the midface?

This is a very common question in my practice. The midface is the area just beneath the lower eyelids; if you draw a line from the corner of the upper lip to the top of the ear, the midface lies above this line. A traditional facelift cannot elevate the midface. This is why some patients have the lateral sweep deformity (the situation where the lower face is pulled and the midface is droopy). However, a deep plane facelift can lift the midface and the lower face together thereby avoiding the pulled look. Although many surgeons do a midface lift separate from a facelift, in my opinion this is of no benefit. A well done deep plane facelift will elevate both these areas harmoniously and produce excellent rejuvenation of the face. What are the characteristics of an attractive eyebrow – Part 2

In the last post I talked about the two components of an attractive brow: 1. Brow shape 2. Brow height We covered brow shape in our last post. Today we will look at brow height. But before we get into this, the viewer needs to understand what the lid cheek junction is. This is the junction point of the lower eyelid and the cheek. It is marked by the single arrow in the first photo on the patients right side (our left). The lid cheek junction appears lower with advancing age and a bulging lower eyelid. A lower lid lift will bring this junction up. In someone with an attractive eyebrow, the pupil is exactly in the middle of two points; one point placed over the mid eyebrow above and the other point placed at the lower eyelid/ cheek junction below. Look at the left eye of the patient in photo 1. The line from the pupil to the brow is shorter than the line from the pupil to the lower lid cheek junction. In photo 2, which shows the patient after a brow lift, the lines are of equal length and the pupil is in the middle of the two points discussed above. With age, the lid cheek junction gets lower. To balance this out, the brow needs to be lifted so that the pupil is still at the middle of the two points discussed above. Conversely, if a lower eyelid lift is done and the lower lid cheek junction is moved up, then the brow will look attractive even without a brow lift. Unfortunately a brow lift is one of the most over done procedures in plastic surgery; this makes the patient look odd, surprised and perplexed as the brow is lifted too high and the inner end of the brow (that is ideally lower) is lifted higher than the outer end (which is ideally higher). Finally between brow shape and brow height; shape takes precedence. A low brow that is well shaped will still look attractive. But a brow at the right height will not look good if the shape is compromised. At Solomon Plastic Surgery, we go for the natural and rejuvenated look. In certain patients, Dr. Solomon does a lower eyelid lift and fat injection to the brow as opposed to a brow lift, which can lift the brow too high. All brows have two components; brow shape and brow height. These need to be in balance for brows to look attractive. In our blog today, we will address the first of these, namely brow shape 1. An attractive eyebrow has a gentle arch such that it peaks at the junction of the inner two thirds and outer one third (upper end of vertical line in the photo) 2. The inner end of the eyebrow is always lower than the outer end of the brow (note the horizontal line and how the inner end of the brow is at the line whereas the outer end is above this horizontal line) 3.The peak of the brow ( upper end of vertical line) when traced vertically down is about 2 to 3 mm inside of the outer corner of the eye (see where the vertical line intersects the eye) Brows look misshaped after a surgical brow lift when one of these happen: 1. The arch is not present or if present, the high point of the arch is not at the junction of the inner two thirds and outer one third 2. The brows are raised too high (What constitutes a high brow? will be discussed in detail in part two of brow aesthetics that will be posted soon) 3. Do I need a breast lift or can I just do implants?

This is a very common question in my practice. A breast lift is an excellent operation but leaves scars on the breast surface. Typically there is a circular scar around the areola, a vertical scar over the middle of the breast and possibly a horizontal scar over the underwire area (lower breast fold) at the base of the breast. An augmentation is usually done through a 3.5 cm scar well placed over the lower breast fold and when healed is almost invisible. Hence the question; can I do a augmentation only without a lift? The location of the lower breast fold never changes. Some women have a high set breast meaning the breast is located closer to the collar bone while others have a lower set breast located further away down the torso. This relative position cannot be changed by surgery. However, what changes is the relative position of the nipple on the breast mound. Typically the nipple is a few centimeters above the lower breast fold and over the center of the breast. With age and pregnancy/lactation related changes, it droops and assumes a much lower position. In such a person, if an augmentation alone is performed, the breast tissue and nipple appear to fall of the implant producing a very unaesthetic look. In other words, the implant appears higher on the chest with the breast tissue falling/drooping off it. Hence a lift is performed to re-center the nipple followed by placement of an implant. In women with larger breasts, a lift alone with re-arrangement of breast tissue will reshape the breasts well without need for implants. Finding out if you have breast ptosis (droop) Place a tape measure under one breast such that it lies horizontally in the lower breast fold; now pass the other end over the surface of the second breast while still maintaining the horizontal lie of the tape measure. If the nipple on the second breast (the breast with the tape measure across its surface) is below the level of the tape measure, then a breast lift is required. Should I go for saline or gel implants?

Both implants have been around for decades and have an established safety profile. Gel implants were taken off the market between 1992 and 2006 as there were reports of possible side effects. However, they were reintroduced when studies did not show correlation with autoimmune disease. Saline implants are essentially a shell filled with dilute salt water! They feel less natural and ripple more. Gel implants feel very natural and soft like breast tissue and ripple less. The older gels were similar to a thick liquid and had the potential to seep into breast tissue when the implant was ruptured. The newer gels are cohesive and do not freely flow like a liquid if the implants rupture. Besides, there is a more cohesive version of gel implants that are shaped; these are called anatomical or gummy bear implants. There is also a newer saline implant that has multiple shells inside which gives it a softer feel that is closer to that of a gel implant. Besides, there are many versions of gel implants that are filled differently and have a varying thickness of gel giving more fullness to the upper part of the implant. The choices are numerous and can be tailored to fit an individual patient. After looking at these photos, can you guess who had a Tummy Tuck? 1) Look at the belly button. Does it have an unusual or “operated” look? 2) An attractive abdomen is flat, but still has highlights. Unfortunately, most Tummy Tucks performed using traditional techniques are overly flat and one-dimensional. 3) Can you see the scar? A well done Tummy Tuck scar should be concealed, even with a low-cut undergarment. (Guess what? All three have had a Tummy Tuck by Dr. Solomon!)
