DR. SOLOMON'S APPROACH

Blepharoplasty

Mayo Clinic-trained plastic surgeon Dr. Matthias Solomon is proud to offer blepharoplasty at his Frisco and Celina offices. As the only surgeon in the Dallas-Fort Worth metroplex certified in both Plastic Surgery and Facial Plastic Surgery, Dr. Solomon developed advanced surgical techniques to enhance his patients' natural features and quicken the healing process.

Blepharoplasty can address sagging skin on the upper eyelids, puffiness under the eyes, or even vision obstruction caused by excess tissue. If you're ready to brighten your look with minimal downtime, Dr. Solomon's techniques are designed to help you look and feel your best.

Blepharoplasty, or cosmetic eyelid surgery, enhances the appearance of the eyes by removing sagging skin, excess fat, and muscle from the eyelids. As we age, the muscles supporting our eyelids weaken, causing the skin to stretch and fat to gather. This procedure is customizable to address these concerns in the upper, lower, or both eyelids for a rested and natural-looking result.

Any complex plastic surgery needs to begin with an in-depth consultation to ensure safe and predictable outcomes. During your initial visit, Dr. Solomon will:

  • Assess Your Anatomy: Evaluate your eye anatomy, skin elasticity, adjacent bone support structure, and the extent of any depressions or dark circles.
  • Review Your Health: Thoroughly review your medical history, current medications, and lifestyle habits.
  • Set Realistic Goals: Review before-and-after photos of actual patients to help you develop reasonable aesthetic expectations and ensure you understand exactly what the procedure can achieve.
  • Customize a Surgical Plan: Develop a personalized treatment plan based on whether you need upper eyelid surgery, lower eyelid surgery, or a combination of techniques.

Proper preparation is the key to a smooth surgery and a stress-free recovery. Dr. Solomon's team will provide detailed guidelines, which typically include:

  • Medical Pauses: You will be asked to stop taking blood-thinning medications (such as Aspirin and NSAIDs), as well as Vitamin E and certain herbal supplements weeks prior to surgery.
  • Lifestyle Changes: Smokers must completely discontinue smoking and nicotine use for several weeks before and after the procedure to promote healthy healing and minimize scarring.
  • Logistics: Arrange for a responsible adult to drive you to and from the accredited surgical center, and have someone available to assist you with initial aftercare for the first few days.
  • Prepare a “Recovery Kit”: Before your surgery day, stock your home with cold compresses, your prescribed ointments and eye drops, gauze bandages, over-the-counter Tylenol (acetaminophen), and extra pillows so you can comfortably sleep with your head elevated.

It's important to understand what blepharoplasty can and cannot address:

  • Upper Blepharoplasty: Improves hooding, removes excess skin, and relieves heaviness but does not elevate the eyebrows or address Crow's feet.
  • Lower Blepharoplasty: Reduces puffiness and improves the lid-cheek transition but does not correct fine lines, Crow's feet, or sagging cheeks.
  • Dark circles may improve slightly as fat redistributes but will not be fully eliminated.

Dr. Solomon can discuss complementary procedures, such as a brow lift or skin resurfacing treatments, for a more comprehensive rejuvenation.

What to Expect
at Your Consultation
Any complex plastic surgery needs to begin with an in-depth consultation to ensure safe and predictable outcomes. During your initial visit, Dr. Solomon will:
  • Assess Your Anatomy: Evaluate your eye anatomy, skin elasticity, adjacent bone support structure, and the extent of any depressions or dark circles.
  • Review Your Health: Thoroughly review your medical history, current medications, and lifestyle habits.

  • Customize a Surgical Plan: Develop a personalized treatment plan based on whether you need upper eyelid surgery, lower eyelid surgery, or a combination of techniques.
Understanding the
Procedure Techniques
Dr. Solomon offers comprehensive eyelid rejuvenation, utilizing advanced, tissue-sparing methods to produce natural, harmonious results without looking "hollowed out" or "overdone." Blepharoplasty is generally performed as an outpatient procedure using either local anesthesia with IV sedation or general anesthesia, depending on the complexity of your surgical plan.

Anesthesia

Blepharoplasty is generally performed as an outpatient procedure. Depending on the complexity of your surgical plan, it will be performed using either local anesthesia with IV sedation (to keep you relaxed and comfortable) or general anesthesia.

Upper Eyelid Surgery & Ptosis Correction
  • Incision Placement: Discreetly located in the natural upper eyelid crease to ensure scarring is well concealed.
  • Targeted Removal: Carefully eliminates excess skin, fat, and, when necessary, a small section of the orbicularis oculi muscle.
  • Ptosis Correction: If your upper eyelids severely droop close to your pupils (ptosis), the procedure can be combined with muscle repair to strengthen the underlying weak muscle.
  • The Results: Reduces hooding, relieves eyelid heaviness, and creates a smooth, youthful contour.
Lower Eyelid Surgery
  • Incision Placement: Made either inside the eyelid or just underneath the lower lash line.
  • Fat Redistribution: Instead of simply removing fat, it is repositioned into the upper cheek area to achieve a smooth, natural contour.
  • Muscle Preservation: Unlike traditional techniques, Dr. Solomon actively avoids cutting the orbicularis muscle, which significantly reduces complications and promotes better, faster healing.
  • Advanced Support: For cases of advanced aging, a specialized tacking suture is placed to provide the eyelid with additional structural support.
Transconjunctival Blepharoplasty

If you do not have severe sagging skin but are bothered by puffy under-eye bags, Dr. Solomon may perform a transconjunctival blepharoplasty. This less-invasive technique involves a tiny incision placed entirely on the inside of the lower eyelid to exclusively remove or reposition excess fat pockets, resulting in zero external scarring.

Learn about
Recovery & Aftercare
The recovery process for blepharoplasty is usually quick, allowing most patients to get back to their regular activities within two weeks. As an outpatient surgery, blepharoplasty allows you to recover at home the same day. Mild swelling and bruising are common afterward and can be managed with ice packs. A prescribed eye ointment will help keep the eyes moist during healing.

To ensure optimal healing, follow these aftercare tips:

  • Apply cold compresses to reduce swelling.
  • Keep your head elevated while sleeping.
  • Refrain from intense physical activity for a minimum of two weeks.
  • Use prescribed eye drops or ointments as directed.
  • Shield your eyes from sunlight with sunglasses.

Recovery typically takes 7–10 days, with noticeable improvements appearing by the third week. As the swelling continues to subside, your eyelids will look more refreshed and youthful. Full results will develop over the next 3–4 months as your eyelids fully heal and settle.

Dr. Solomon's team will offer comprehensive aftercare instructions to help you heal comfortably and achieve optimal outcomes. Follow-up visits will ensure your recovery progresses smoothly and any concerns are promptly addressed.

FAQs
How long do the results of blepharoplasty last?

Blepharoplasty results can last 10 years or more, and for some, the results are permanent. While the natural aging process continues, the removed excess skin and fat do not return.

What are the risks of eyelid surgery?

While highly safe in the hands of a board-certified facial plastic surgeon, any surgery carries risks. Minor, temporary side effects can include dry eyes, light sensitivity, blurred vision, or watering eyes. Severe complications, such as hematoma (blood collection under the skin) or ectropion (downward pulling of the lower lid), are exceedingly rare due to Dr. Solomon's conservative, tissue-sparing techniques.

Will blepharoplasty change the shape of my eyes?

No. Blepharoplasty enhances your natural eyelid contours without drastically altering your eye shape. Dr. Solomon's precise techniques ensure a refreshed look while preserving your unique facial harmony.

Will I have visible scars after eyelid surgery?

Incisions are placed in the natural eyelid creases or completely inside the lower eyelid, making scars barely visible. With proper healing and sun protection, these fine lines fade significantly over time.

How much does blepharoplasty cost?

The cost of blepharoplasty varies based on the extent of the procedure (upper, lower, or both) and individual anatomical needs. Dr. Solomon will provide a transparent, personalized treatment plan and pricing breakdown during your consultation. Financing options are available.
