Your explant surgeon should ideally have experience in en bloc breast implant removal procedure. En bloc removal refers to the surgery where the surgeon takes out the scar tissue (or capsule) and breast implant simultaneously, with the implant still inside the unbroken scar tissue capsule. This is vital if your breast implants are filled with silicone gel, are damaged, or are very old. 

En bloc capsulectomy lets the silicone and other harmful chemicals stay inside the unbroken scar tissue capsule while it is being removed. The technique is designed to prevent silicone, bacteria and other chemicals from seeping into the woman’s body during the removal process. 

En bloc capsulectomy may involve a slightly bigger incision (which can mean a relatively bigger scar), but it is worth it because the procedure checks silicone leakage that could lead to several health problems.  

Bear in mind that en bloc procedure may not be undertaken in all cases, and your surgeon may not know if an en bloc removal can be performed until you are in surgery. For instance, if your scar tissue capsule is weak or attached closely to the wall of the chest, en bloc removal may not be recommended.
